Athlete Consultation
Service Description
Grades: Rising 7th-8th Rising 9th-11th Senior (12th) Post High School An initial consultation is required to book any further services with CMJ Performance & Sport Science. The meeting includes an in-depth evaluation of the athlete’s current situation and desired goals. This session provides a detailed snapshot of the program(s) needed to develop the athlete profile within our academy program. Enrollment Credit Offer: If the athlete enrolls in a program within 15 days of the consultation, the full consultation fee will be credited toward their program fees. If enrollment occurs within 30 days, half of the consultation fee will be credited toward their program fees. Cancellation Policy
Lateness & No Show Policy: It is required that consultation sessions start on schedule. A session will be forfeited if you arrive 15 minutes late for a 45-minute session. If the client is late, the CMJ Performance & Sport Science reserves the right to leave the consultation session. Unless prior arrangements have been made with the assigned coach, clients who arrive after their scheduled session has begun will only receive the time that is left. If the coach is running late, the client shall be owed back the missed time at a time suitable to both the coach and the client. Cancellation Policy: Please contact the coach by email and/or other mutually agreed-upon means of communication at least 24 hours in advance if you are unable to show up for a scheduled coaching session; failing to do so will result in full payment for the session. At the coach's discretion, modifications may be made, and appointments may be rescheduled. A session is due to the client if the coach cancels it. Payment Policy: Payments are due prior to the scheduling of solution(s). For School Districts & Private Schools: Purchase Order (P.O.) must be submitted to CMJ Performance & Sport Science prior to scheduling of solution(s) and must be paid within 30 days of the completion date of the first solution. Expiration Policy: Solution(s) must be used within 30 days of the purchase date. After this time, solution(s) and packages become invalid. There is no transferability for any solution(s) or package(s). Package(s) and solution(s) are not refundable. Refund Policy: In cases of injuries or changes in medical condition leading to physical limits, exceptions may be provided at the coach's discretion (a doctor's note is required) for unused solution(s) or program(s). Complaints Policy: If for any reason you are unhappy with the coach, you may transfer to another coach at any given time. In all circumstances, any sessions used with the previous coach remain used. The coach cannot prescribe treatment or diagnose medical conditions. They may at any time ask you to consult with your doctor should any mental health or medical condition arise.
